Краткий осмотр (реферат): Актуальність. Остеопороз є актуальним питанням сьогодення, оскільки впливає на якість і тривалість життя хворих, підвищує ризик низькоенергетичних переломів, що призводить до значних медичних, соціальних і економічних проблем. Однією з причин розвитку вторинного остеопорозу є застосування антагоністів вітаміну К і нових пероральних антикоагулянтів, використання яких зростає. Метою цього систематичного огляду був аналіз літературних даних стосовно впливу пероральних антикоагулянтів на стан кісткової тканини, ризик розвитку остеопорозу та його ускладнень з метою поліпшення поінформованості лікарів різного фаху. Матеріали та методи. Огляд наукової літератури проводили на основі аналізу міжнародних електронних наукометричних баз даних PubMed, Google Scolar, Scopus за період 2013–2023 рр., але з нього не виключали основні роботи, опубліковані раніше. Пошук здійснювали два незалежні автори. Для аналізу було відібрано 160 джерел, з них використано 53, які відповідали критеріям пошуку. Результати. Аналіз наукових робіт продемонстрував, що вплив антагоністів вітаміну К і нових пероральних антикоагулянтів на кісткову тканину та ризик розвитку низькоенергетичних переломів суперечливий. При порівнянні антагоністів вітаміну К і нових пероральних антикоагулянтів показано, що при використанні останніх спостерігали менший ризик розвитку малотравматичних переломів. Висновки. Більшість досліджень продемонстрували несприятливий вплив антагоністів вітаміну К на стан кісткової тканини. При прийомі нових пероральних антикоагулянтів спостерігали нижчий ризик переломів, ніж при використанні антагоністів вітаміну К, зокрема варфарину. З огляду на проаналізовані дані літератури рекомендовано при призначенні антагоністів вітаміну К чи нових пероральних антикоагулянтів також оцінювати ризик виникнення низькоенергетичних переломів. Background. Osteoporosis is actually today's problem, as it affects the quality and length of life, and increases the risk of fragility fractures, which leads to significant medical, social, and economic problems. One of the reasons for the development of secondary osteoporosis is the use of vitamin K and new oral anticoagulants, the use of which is increasing. The purpose of this review was to analyze of literature data regarding the impact of oral anticoagulants on the bone state, the risk of osteoporosis and its complications in order to improve the awareness of doctors of various specialties. Materials and methods. The literature review was carried out based on the analysis of scientometric databases PubMed, Google Scholar, and Scopus using the keywords “vitamin K antagonists”; “warfarin; “osteoporosis”; “osteoporotic fracture”; “non-vitamin K antagonist oral anticoagulants”; “systematic review” for the period 2013-2023, but it did not exclude key works that were published earlier. The search was performed by two independent authors. 160 sources were selected for analysis, of which 53 were used that met the search criteria. Results. The analysis of researches showed that the effects of vitamin K antagonists and new oral anticoagulants on bone and the risk of fragility fractures are controversial. Nevertheless, the new oral anticoagulants in comparison to vitamin K antagonists seem to have a less negative effect on bone and the lowest risk of fragility fractures. The mechanism of such influence is still under examination. Conclusions. Most studies have demonstrated the adverse effects of vitamin K antagonists on bone health. A lower risk of fractures was observed with new oral anticoagulants than with vitamin K antagonists, in particular, warfarin. Taking into account the analyzed data of the literature, it is recommended to also assess the risk of fragility fractures when prescribing vitamin K antagonists or new oral

Краткий осмотр (реферат): Introduction: Takayasu arteritis (TAK) is a granulomatous inflammation of vessels of large diameter,
mainly affecting the aorta and its proximal branches, which is more common in young women.
The incidence of coronary artery disease in TAK is unknown and not sufficiently studied.
Material and methods: A literature review was performed for the period 2003 to 2023 using a search
of the PubMed and Scopus scientific databases. We used the following key words to search the academic journal databases: “Takayasu arteritis”, “coronary angiography”, “myocardial revascularization”, “coronary artery disease”, “angina pectoris”, and “myocardial infarction”.
Results: The analysis of studies showed that coronary artery lesions (CAL) in TAK are not uncommon,
being identified particularly in children and young individuals. Pathology of the coronary arteries
increases the morbidity and mortality of TAK patients. In TAK, the ostia and proximal segments
of the coronary arteries are more frequently affected, presenting as stenosis or occlusion with the development of stable coronary artery disease, unstable coronary artery disease, acute myocardial infarction, and sudden death. The diagnosis of TAK and coronary artery pathology is often complicated due to nonspecific manifestations and frequently the absence of symptoms. The treatment of patients with TAK with CAL is challenging and requires both pharmacological therapy and interventional or surgical methods of myocardial revascularization.
Conclusions: Takayasu arteritis is a more prevalent cause of CAL development, especially in young patients, than previously believed, and cardiovascular death due to CAL is not rare. Early diagnosis of TAK
with CAL, particularly occlusion or hemodynamically significant stenosis, and the prompt initiation
